1. Fill up all the necessary information and submit the
completed form in any of our Digiprint branch.
2. Important details need to be accomplished are the following:
a. Full Name
b. Complete Addresses
c. Contact Numbers and Email Addresses
d. Member
Association, School of Photography Magazine Subscription, address and contact
number
e. Signature
3. Submit the following requirements:
a. Photocopy of valid IDs (company ID, SSS/GSIS, Driver’s License, Passport)
b. Photocopy
of photography membership ID or photography school ID or certificate.
c. If magazine subscriber, photocopy of proof of billing.
4. Have your 1x1 ID photo taken at any Digiprint branch.
5. The application form will be sent to Digiprint Head Office for screening and
verification of the correctness of
the details. Digiprint will inform the photographer via phone or email to send
any information or documents
needed for final approval.
6. If no call was received it means that application is approved.
7. Digiprint PRO membership card will be delivered within 14 working days.
8. ID contains name, 1x1 ID picture and signature of member.
9. As soon as the member receives the card, he/she must always present the membership
card and valid IDs
upon transaction at any Digiprint branch.
10. Digiprint maintains a centralized monitoring system of all transactions made
by the Digiprint Pro members. |

c. Photo Gift Items - 3-5 working days
d. Prints and Photo Gift Items - 3-5 working days |

2. Payment must be made either in cash or credit card
as soon as the Digiprint Pro member transacts with Digiprint.
3. Customer shall pay in full. |
